Start by registering here and filling out the form with your practice information.
Once your account has been created, proceed to log in.
Under the
Orders
tab, click “Add Order” and fill out the prescription form for the patient and choose which service type and add-ons you would like. NOTE: During this step you may also upload patient bite photos which will assist us in more accurately setting the occlusion.
Click “Save” and then “Print Order Form”
Prepare impressions for shipping by placing them in a sealed plastic bag along with a wax bite or bite register.
NOTE: We recommend using a PVS material (polyvinyl siloxane) for all impressions and bite registers. If using
alginate impressions, please wrap them in a damp paper towel before sealing them in the plastic bag. With alginate
impressions, some model distortion may occur. If using a wax bite register, it MUST be non-translucent in order to
achieve proper occlusal placement.
Request a shipping label here. The shipping label will be emailed to you.
Attach each order form to its corresponding bag of impressions and place in the box. Apply shipping label and drop off at post office.
